Introduction
Continuity of Care is a data set that can be transfer from on
HCP to another HCP to support the continuity of care of the Client. It includes
the most relevant facts about a client's healthcare.
The benefits are;
- Provides for the receiving HCP to have easy access to all
information needed to continue the care of the Client. Some of the contents,
but not limited to, are:
- Demographics
- Problems
- Allergies
- Relevant history
- Laboratory Results
- Assessment Results
- Subjected follow up care
- Enhance patient safety by:
- More accurate information because the receiving HCP
can ask the same questions to verify that the information that is
received is correct.
- Verification of Allergies
- Verification of Medications
- Receiving HCP can "fill-in" missing information
- eliminating duplicate medication prescriptions

- The receiving HCP does not have to write down the
same information again
