Introduction

Case Management in the InHCc HMIS is a different concept then normally Defined.

Case Management in InHCc is the longitudinal care of "Every Client." Each Client is assigned a team of healthcare professionals that is "always" responsible for that client wither it is the for the current visit or the clients health in future. The Case Management Team is responsible for ALL Risk factors that the Client may have. There is no "Discharge" in the InHCc System. The Client is always under the care of the Case Management team. 

Case Management in the InHCc system

InHCc uses Evidence based “Protocols” that are built into the system. By selecting a “Problem” a Standards developed treatment plan can be created.

Case Management is the continuous care of the Individual, either in the healthcare organization or external. The process is:

  • Case (for every patient)

  • Case Management

  • Assessments based on "standard protocols"

  • Development of a Problems/Diagnosis list

  • Treatment Planning based on "standard protocols"

    • Medication

    • Screening Examinations

    • Patient and Caregiver Education

  • Order Management

  • Assignment of Resources

  • Implementation of the Plan (Procedures)

  • Patient Safety

    • Clinical Alerts based on “Rules” that can be modified

    • “Required” data entry field

  • Utilization Review

    • Measure outcome

    • Evaluate outcome and resources used (Case/Utilization Review)

    • New examination and plan

  • Development of a "New" Plan
