Introduction
I once had a Director of a major NGO warn me personally
about trying to implement a new information system. He said “if you make any
changes to any of the existing systems, you will find someone at your door that
will do you serious body harm.” Now either the Director was warning me to be
careful of others, in which regards he knew that these occurrences where taking
place, or he was warning me that he would do something. We will give him the
benefit of the doubt.

Could not stop it
It seems that the management was so bad that no one
wanted anyone else to know, including the management themselves. They told so
many lies, they did not themselves know where they stood.
People will take advantage of any situation that comes their way. It is not
enough to say that "Oh, they wouldn't do that."
Keep them honest...or at least make them work for it.
To create a post required
by a Donor and recommended by an internal review team of the Organization, it
was recommend that the post of Internal Auditor be established. InHCc was to
help set this department up, establish procedures, and hire and train an
individual to fulfill this job. The job had previously been advertised in the
paper, but mysteriously the advertisement was pulled out of the paper and the
responses to that ad disappeared. The salary for this position had even been
included in the budget. However, since this Internal auditor was to report
directly to the Director and the Board of Trustees and not be a part of the
finance department, the Financial Director refused to let this position be
established...[I wonder why?]
In one organization:

food purchases was bought by one person with no control on the prices he
paid.

A
random check of prices for office supplies show that the price the BNGO
was paying 20-50% more than the local prices. The reason that was offered
for this situation was that the "prices was set once a year and it
was set high enough to cover the vendors risk." ...kind of makes you
want to be the vendor!
